I have some interesting stuff to add here:
Using meteor (or stone grip) on a target area in FRONT of an enemy but not including the enemy then waiting for the enemy to enter the spell will almost always hit.
targeting enemies that are on the very edge of your sight and have not yet seen you will almost always miss.
For boxes or other destructible objects placing the object in the centre of your target will cause it to often miss, while placing it just outside your target will cause it to hit almost every time.
as for all skills missing in this way....
nightmare has NEVER missed for me, neither have staff or bow ranged attacks (not skills)
close-range attacks such as swords, daggers, etc often miss barrels, crates, etc.
ranged magic other than meteor and stone grip rarely miss but occasionally do.
personally I think these are down to the game not loading certain parts of enemies (such as their collision box) quickly enough.
bear in mind that dungeon hunter was originally a mobile phone game, even though this version is PS3 exclusive it probably uses the same engine as the other versions.
the PS3 fails at emulating mobile phones (just look at the "angry birds" and "zenonia" mobile ports on PSN)
